Incident referrals (city care homes only)

Please ensure you have read the Notifiable Incidents list (link) to decide whether you need to report this to the Local Authority before proceeding any further. If a decision is made that the incident is notifiable you then have two options for reporting it via the portal using either the ASC or CaAS incident reporting form. The Notifiable Incidents List will guide you on which form to use.

Always ensure that you provide as much information as possible to support the Local Authority to decide quickly if further action is needed. We aim to respond to the online form in 10 working days if action is required.
