If this is the first time you have contacted us about yourself or someone you care for, please answer some questions to enable us to find out more about your circumstances. You may have difficulties with everyday tasks, due to age related frailty, have a disability, a mental health problem, a sensory loss, an illness or a condition such as autism.

We will ask you questions about how you do things like:

  • Getting washed and dressed
  • Using the toilet
  • Making and eating meals
  • Moving around your home safely
  • Keeping your home clean and safe
  • Getting out and about
 
Based on your answers we will provide you with information where you may find and arrange the support yourself.
